Pediatric Patients and Adults with Strabismus
Follow Up Visits and Medical Records
Dr. Leung now sees her pre-existing pediatric patients and adult patients with strabismus (misaligned eyes) at the new Scarborough office location. There will no longer be a pediatric ophthalmologist based in Oakville after April 6, 2020. If you have or your child has an upcoming follow up appointment with Dr. Leung scheduled after April 6, 2020, the office will contact you with a new appointment date and time at the new location.
Medical records will automatically remain with Dr. Leung unless the office receives signed and written authorization to transfer a faxed copy of the records to another physician (free of charge). This authorization form can be downloaded by clicking the button below and can be mailed or faxed by your physician to our office. Copying, printing or digital reproduction of medical records will be subject to the standard Ontario Medical Association rate for uninsured services.
If you wish to continue your child’s/ your care with a different physician, it is advisable that you find a new ophthalmologist for future care as soon as possible. There are three ways to do so:
1) You can contact your family physician.
2) You can conduct a “doctor search” on https://aapos.org or https://www.cpso.on.ca (links below).
3) You can contact Health Care Connect at 1-800-445-1822 for information on other ophthalmologists.
Please note that Dr. Shannon Daniel, who covered for Dr. Leung during her maternity leave in 2019-2020, does NOT see pediatric patients or patients with strabismus.
