Adult Patients

Dr. Shannon Daniel has taken over the care of all active adult patients who are NOT being seen for strabismus (misaligned eyes). Dr. Daniel received her ophthalmology training at McGill University and completed her residency in 2012. Many existing adult patients have already met Dr. Daniel, who covered for Dr. Leung during her maternity leave. Dr. Daniel has taken over Dr. Leung's previous office space at 710 Dorval Drive, Unit 701, Oakville, Ontario and will be seeing adult patients at that location.

If you are an adult patient previously under the care of Dr. Leung and you have an appointment scheduled, you do NOT need to do anything to transfer your care to Dr. Daniel. Dr. Daniel's office will call you to confirm the date and time of your existing appointment closer to the appointment day.   

Your medical records will automatically remain with Dr. Daniel unless Dr. Leung's office receives signed and written authorization from you to transfer a faxed copy to another physician (free of charge). This authorization form can be downloaded by clicking the button below and can be mailed or faxed by your physician to Dr. Leung's office. Copying, printing or digital reproduction of medical records will be subject to the standard Ontario Medical Association rate for uninsured services. 

If you wish to continue your care under a different physician, it is advisable that you find a new ophthalmologist for future care as soon as possible.  There are three ways to do so:

3) You can contact Health Care Connect at 1-800-445-1822 for information on other ophthalmologists in your area.
